Hosparus Health & Nazareth Home partner to launch ‘HELD: Helping Empower Life’s Decisions’
In collaboration with Nazareth Home-Clifton, Hosparus Health has launched a nurse practitioner consult program, HELD: Helping Empower Life’s Decisions, to enhance care and improve quality of life for seriously ill residents of the senior care community.
A HELD consult is designed to explore and establish the resident’s goals for care, improve care coordination and collaboration, provide expert symptom management, assist with advance care planning and provide patient and family education.
In the program, which began Oct. 16, 2017, specialty palliative care consultations are provided by Hosparus Health nurse practitioners. The program is currently being offered exclusively at Nazareth Home’s Clifton location, 2120 Payne St.
“Hosparus Health has partnered with Nazareth Home for many years to provide traditional hospice services to residents who are expected to live six months or less,” said Lori Earnshaw M.D., Hosparus Health Medical Director. “This new consult program brings the hospice philosophy of interdisciplinary, holistic and patient-centered care to patients living with the burden of chronic illness and can be provided at any stage of a serious illness. Advanced illness care, also called palliative care, optimizes quality of life for seriously ill patients by anticipating, treating and even preventing disease symptoms. Our goal is to help more residents sooner, easing their suffering and making the most of their golden years at Nazareth Home.”
